Forms to Take to Your Peditrician

Here are a number of emergency care plans that you can take to your child's physician. These will be kept in their permanent health record for the duration of one school year. Each care plan will help our staff manage an emergency if necessary according to you and your physicians direction. Please call your school nurse if you have any questions.
