Forms to take to your Health Care Provider

Injuries can happen at any time.

The following forms can be copied and brought to you child's Health Care Provider thoughout the school year. This will allow us to be up to date on any health issues your child may have. Please return these forms to your child's School Nurse or Health Office as soon as possible.

Dental Health Certificate (pdf file - 123 KB)
NEW! NYS requests dental certificates for students in grades K, 2, 4, 7 and 10.

Health Certificate/Appraisal Form (pdf file - 96 KB)
This form can be used for annual school physicals as well as sport physicals.

Physical Education Adaptive Participation Form or Medical (pdf file - 35 KB)
NYS requires all students to to attend regular or adaptive physical education. If your child will need to be out for more than two days, please take this form to your child's physician to be completed.
