Our Family ID sports registration opens on October 3,2023 for modified sports and October 13, 2023 for JV and Varsity. Family ID is a required part of participation in athletics and must be completed in addition to completing a physical every year. Prior to being able to participate in athletics, you must have an up-to-date physical on file in the health office and be approved in Family ID. 

New York State mandates that an annual physical completed within 12 months prior to the start of practice be on file at the student athlete’s school. If your child has not had a physical in the past 12 months, please contact their PCP (Primary Care Physician) as soon as possible to schedule an appointment. In the event that your child is without insurance or other factor that delays a physical being completed prior to the first day of practice, our School Medical Director may be able to do the physical at Clyde-Savannah High School. 

Any student who has an underlying health condition or taking routine or rescue medications MUST see their PCP for the annual exam. If an athlete is prescribed rescue medications (ie: inhalers, EpiPens), must be seen by own PCP who can prescribe medication. Medication orders for inhalers and EpiPens must be written allowing students to “carry and self administer”. The expectation is they are comfortable using medication on their own and will carry this to practices and sporting events.

In order to participate, students must be approved with Family ID. Once registered students are placed in a Q for approval, they are processed in the order in which they have been received. 

 Family ID takes your entry and auto populates for all forms and multiple programs across our system.
